RN Case Manager (Temp to Perm)<br />Location: Glasgow, KY<br />Payment: $55/hr<br />Schedule: Mon-Fri 8-5<br /><br />Shift Requirements<br /><br />Monday‑Friday 8am‑5pm.<br /><br />On call every 3rd weekend.<br /><br />1‑2 nights during the week.<br /><br />License Requirements<br /><br />Compact Registered Nurse license.<br /><br />Benchmark Requirements<br /><br />30 points per week.<br /><br />Mileage Reimbursement<br /><br />0.43 USD per mile.<br /><br />County Covered<br /><br />Allen<br /><br />Barren<br /><br />Hart<br /><br />Preferred Skills<br /><br />HCHB experience preferred.<br /><br />Wound care.<br /><br />IVs.<br /><br />Primary Responsibilities<br /><br />Perform comprehensive assessment of high‑risk members.<br /><br />Collaborate with primary care providers to implement individualized, comprehensive care plans with specific interventions designed to engage the member.<br /><br />Address, identify, and continuously reassess cost‑efficient, appropriate levels of care.<br /><br />Put non‑medical support in place to ensure compliance with treatment plans (e.g., housing and transportation).<br /><br />Coordinate care transitions as needed.<br /><br />Coordinate with physicians, social workers, discharge planners, and claims professionals in transitioning a patient to the appropriate level of care.<br /><br />Engage specialty resources as needed to achieve optimal resolution.<br /><br />Keep detailed records of clinical, functional, and fiscal outcomes during the management process.<br /><br />Identify opportunities for health promotion and illness prevention.<br /><br />Prevent adverse patient occurrences when possible, and intervene quickly if prevention is not possible, thereby minimizing poor outcomes.<br /><br />Act as patient advocate by protecting privacy and confidentiality issues.<br /><br />Qualifications<br /><br />Associate's or Bachelor's degree in Nursing or related field.<br /><br />Current, unrestricted state Registered Nurse license.<br /><br />Ability to analyze complex medical information and make rational decisions.<br /><br />Case Management certification.<br /><br />Certified Case Manager (CCM) required.<br /><br />Must pass background check, drug screen, and have updated immunizations.<br /><br />#J-18808-Ljbffr
